Understanding the Impact of Social Isolation and Loneliness Programs Community of Practice

 

Purpose 

This community of practice involved three organizations with programs or interventions that address social isolation and/or loneliness and that utilize the Upstream Social Interaction Risk Screener (U-SIRS) as a “pre-test” and “post-test” to collect data and better understand their program or intervention’s impact. 

Members of the Community of Practice: 

  • Received an introduction to U-SIRS and no-cost access during participation 
  • Learned from a leading researcher and additional guest experts 
  • Received technical assistance and support with implementing U-SIRS into workflow and programming 
  • Grew their professional network with others interested in this topic 

Phase I ran from May through August 2023. Commit to Connect concluded Phase II, which ran from December 2023 to August 2024.

  • Meals on Wheels of Southwest OH and Northern KY (Cincinnati, OH) 

Executive Summary Report 

Commit to Connect developed an Executive Summary Report to provide more details on the strategy, design, member profiles, outcomes, and best practices.
